Non-Member LAMS License

In order to obtain a LAMS License, you must print and fill out the LAMS Licensee Application Form (14k PDF) and the License Agreement for SDA Memory Card Specification (106k PDF). SIGN AND MAIL PRINTED COPIES TO THE SDA OFFICE AT THE FOLLOWING ADDRESS:
